acknowledge that I have voluntarily rented a paddle board from Paddle Board Ohio for my personal use. In consideration of being permitted to rent this paddle board, I hereby agree to the following terms and conditions:

ACKNOWLEDGEMENT OF RISK*

I acknowledge that paddle boarding is a potentially dangerous activity and that there are inherent risks involved in such activity, including the risk of serious bodily injury, death, or property damage. I acknowledge that I am aware of the risks and hazards associated with paddle boarding and that I voluntarily assume all such risks.

RELEASE OF LIABILITY*

I release and discharge Paddle Board Ohio, its officers, directors, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me or any third party as a result of my use of the paddle board, whether caused by the negligence of Paddle Board Ohio or otherwise.

INDEMNIFICATION*

I agree to indemnify and hold harmless Paddle Board Ohio, its officers, directors, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me or any third party as a result of my use of the paddle board, whether caused by the negligence of Paddle Board Ohio or otherwise.

RESPONSIBILITY FOR LOST OR DAMAGED ITEMS*

I agree to pay Paddle Board Ohio for any lost or damaged items that may occur during my rental period, as outlined in the rental agreement. I acknowledge that I am responsible for the paddle board and any associated equipment during the rental period.

TIMELY RETURN OF EQUIPMENT*

I agree to return the paddle board and any associated equipment to Paddle Board Ohio on the agreed-upon date and time. I acknowledge that failure to return the paddle board on time may result in additional fees.
